
How the services are done:
First, you will fill out the form to learn more information. Provider will call you and explain how everything works and answer any questions you may have. This appointment is by phone and will last around 15 minutes.
If you become a new patient, you will be sent via email, all the history intake forms and consent forms which will need to be filled out at least 3 days before your initial new patient appointment. This will give us a better understanding of all your history. We will need any laboratory and/or diagnostic testing you have done in the past 12 months to learn your baseline and trends. We will need a list of all supplements, vitamins, and medications you are currently taking.
Please complete and submit the intake and all other forms sent to you via the practice portal (or email) at least 3 days prior to your appointment time.
Initial first new patient appointment, will go over all your extensive history and discuss previous illnesses, injuries, trauma, stress, diet, lifestyle, and any recent labs in last 6 months and will discuss this information and suggest other lab work that may need to be done such as hormone labs, thyroid labs, nutritional panes, and stool tests, etc. (Typically 90 min. to 2 hours depending on complexity of the history)
New patient follow-up, we will go over the results of your labs and create your personal treatment plan that addresses any root causes of your health issues. This plan may include making adjustments in your diet and lifestyle, suggesting nutraceuticals or supplements, reducing stress, allergens (environmental and food) and reducing toxins. Sometimes, even pharmaceutical medications may be prescribed, for example, thyroid hormones, if needed. (Typically 90 min. to 2 hours-depending on complexity)
Follow-ups, you will have continued follow-ups to check on your progress and help support you on your healthcare journey. At regular intervals, some of the lab testing will need to be ordered again to evaluate the progress of any imbalances, to make sure you are on the right track for your health goals. (30 min to 60 min).
